ONE (1) MCV on the first day of 12th grade. If one dose was given at 16 years of age or older, that shall count as the twelfth grade dose. No child will be admitted to school without providing the appropriate immunization information to the school nurse. Pennsylvania law states a child must have at least one dose of each of the required vaccines in order to attend school on a provisional basis.
